All of the monsters in the first game are obviously manifestations of Alessa, from the Mumblers to her memory of Lisa. However, the doctors and nurses are different in that they seem to be real people who have been converted by parasites which enter and possess them through the spine.

So are the nurses real people who got sucked into the alternate dimension? Is it Alessa or Dahlia that controls the parasites? And, if the nurses are real people, do they rematerialize as dead bodies in the real world once killed, or was Alchemilla simply rocked by a mysterious disappearance of the ENTIRE hospital staff? Who is responsible for Cybil's possession and why? And how come Kauffman never got possessed? Any theories on this strange occurrence would be greatly appreciated.

There is nothing more mentioned about the parasites themselves. From what is gleaned from the above information we can assume that the nurses and doctors are all in the Otherworld. And if I remember correctly, when Harry goes to Lakeside to confront Cybil he enters the Otherworld again.

So the parasites are Otherworld only. As for Kaufmann, I don't remember him ever appearing in the Otherworld, so he couldn't get infected.

In theory, since Kaufmann and Dahlia were at the time the main figures in the cult, and Dahlia is the main cause of the events in V.1, the nurses and doctors from the hospital could have been sucked into the Otherworld as a result of Kaufmann and Dahlia's manipulations.

If the theory of "each person sees their own monsters" holds true, then the nurses and doctors could see Harry as an escaped patient and are trying to put him back in his room, hence the amount of nurses grouped together at one time and the doctors being more forceful. They might see the real world instead of Otherworld and are trying to keep order just as they always would.
