From what I've seen in my own experiences, I always just thought Stanley was a disturbed man with obsessive tendencies. Just because he was obsessed with Alessa/Heather doesn't necessarily mean they were together, at least in my opinion.
As for being "the only sane man", many insane people think they are sane and everyone else is crazy. That's, again, a normal statement of someone who is disturbed and delusional.
