The best explanation I heard for this was on the Black Heliz breakdown of SH, as far as the Twin Peaks connection (the intro being something of a montage of things that have already happened on the "show").
Replacing Harry's wife with Cybil is sort of a hint that this is a false ending; even though you have acheived happiness and everything is now OK, the vicious cycle continues...choosing "Next Fear" just stirs the pot and the whole thing starts all over again. Besides, the Good + ending just seems a little too good to be true for a Silent Hill game
According to one of the game creator's, what happened to Cybil is supposed to be left to our imagination. Since we never heard from her again, it's just sort of assumed that the Good ending, where Harry escaped but he killed Cybil in the process, is the true ending and true escape; thus, it is not seen in the intro. Did that make any sense?
