What do you think it means when Vincent says to Heather, "They look like monsters to you?"
If it's a joke as Vincent says, it's an odd one.
Here is a trancript of the exchange from Gamefaqs:
Vincent: Don’t stand there looking so smug.
Vincent: You’re the worst person in this room.
Vincent: You come here and enjoy spilling their
blood and listening to them cry out.
Vincent: You feel excited when you step on them,
snuffing out their lives.
Heather: Are you talking about the monsters?
Vincent: Monsters...?
Vincent: They look like monsters to you?
*Heather gasps.*
Vincent: Don’t worry, it’s just a joke.

I think the general consensus is that it was a joke. There are numerous interpretations about the general question of "what is reality" in Silent Hill. But as for Heather mistaking innocent victims for monsters, seems unlikely.

That quote always seemed odd to me, but I have always taken it literally. See, I am one of those people that believes that the protaginist could quite possibly be killing other "people". In the world of Silent Hill these "people" are changed into something that could make a connection within the protaginists mind, and that to me is what the monsters could possibly represent. However I still have some doubts, perhaps this info will be revealed in a future game?

If I remember correctly (it's been a while since I've played it) the way Vincent says that it was a joke, his tone and cadence, implies that he wasn't joking.
I'm not sure what to think if he wasn't joking though. Does that mean that Heather is walking around killing people in the real world? Does Silent Hill make Vincent see the monsters as benign, reflecting his mindset? Are there "blank" monsters roaming Silent Hill, allowing each person to see them differently, kind of like a monster Rorschach test? Confused
Is any of this like what you're saying shootzombies?

Yep thats exactly what I was saying. Also if you think about, why would Vincent have such a long conversation with Heather about this subject if he truly was "just joking". It seems to me that he went a bit far with the joke just so he could say- "Oh don't worry about I was just joking". This allows me to come to the conclusion that he truly was not joking.

I'll still maintain that SH monsters were normal folks at one time. I believe Vincent, for whatever reason (long exposure to the environment, being a "monster" in his own right like Claudia) understands this idea or perceives more deeply into the creatures, recognizing a few perhaps for what they once were. Also monster is a loose term, some true monsters look perfectly acceptable. By saying they aren't monsters doesn't mean they appear as human to him, it just means that perhaps beyond the hideous exterior they are inwardly un-monstrous.

We must also take into account that Heather does have the God growing inside her again, which can skew her perception of "reality".
Vincent and Claudia, being the new leaders of the Cult, are forcing Heather to remember who she once was. Combine this with the rest of the creepy outward and inward foils of Silent Hill plus the fact that Heather can create the same does mean that Heather could be killing innocent people.
Non-believers, innocents, women, children, whatnot could all be the victim(s) of a God gone mad.

I thought of that, but they act monstrous, meaning they attack and kill without provocation. Of course, that could simply be a part of Heather's perception/dillusion/whatever. Or were you simply saying that Vincent sees beyond even their actions, recognizing the humanity that once was?
On the other hand, if that's the case, Vincent shouldn't be surprised that Heather doesn't see beyond their "surface" appearance.
